Donating through the Mayo Clinic Arizona
If you wish to donate your kidney through the Mayo Clinic in Phoenix, Arizona - the first step is to register on their site and fill out the Health History Questionnaire.
You can do this at: https://mayoaz.donorscreen.org
A Living Donor RN Transplant Coordinator will review your questionnaire, and if you are a possible kidney transplant donor candidate, they will reach out to you.
